CLINICAL APPLICATION OF PECTORALIS MAJOR MYOCUTANEOUS FLAP FOLLOWING ABLATIVE SURGERY OF MALIGNANT CANCER AT HEAD AND NECK

Abstract

|
We used pectoralis major myocutaneous flaps to cover the head and neck defects following ablative surgery for malignant cancer in 16 cases of 15 patients from Feb. 1985 to Aug. 1988.
This flap should be excellent in the reconstruction of external defect of the head and neck in 7 cases. But, in the reconstruction of intraoral and cervico-esophageal defect, in 5 cases, its bulkiness would give the limitation of tongue movement and speech, give narrowing of reconstructed esophageal lumen.
P articularily, if the recipient bed was massively irradiated or the complex defect was required to reconstruct the mandible with soft tissue, the complication rate would be higher because of poorly vascularized recipient bed. Here, we present our experience and review the literatures about application of this useful flap.
